We are seeking a dedicated Registered Nurse (RN) Case Manager to join a dynamic healthcare team. This role is responsible for coordinating patient care across acute care, swing bed, and outpatient settings, ensuring high-quality outcomes, efficient resource utilization, and compliance with regulatory standards.
The ideal candidate will have strong experience in case management, discharge planning, utilization review, and care coordination.
Key Responsibilities:- Manage patient care across the continuum, including acute care, swing bed, and outpatient services
- Perform discharge planning to ensure safe and timely transitions of care
- Conduct utilization review to support appropriate level of care and cost efficiency
- Collaborate with interdisciplinary teams including physicians, nurses, and social workers
- Ensure compliance with CMS guidelines and regulatory standards
- Coordinate patient services, referrals, and follow-up care
- Advocate for patients to ensure optimal healthcare outcomes
- Maintain accurate and timely documentation using EMR systems
- Active Registered Nurse (RN) license (or willingness to obtain)
- BLS (Basic Life Support) certification required
- Minimum 2 years of clinical or case management experience
- Strong knowledge of care coordination, discharge planning, and utilization review
- CCM (Certified Case Manager) or ACM (Accredited Case Manager)
- Experience working in acute care or hospital-based case management
- Familiarity with Cerner or other EMR systems
