Practice Setting: Outpatient
Reason for Coverage: Short-staffed
Average Number of Patients Per Day: 12
Documentation System/EMR: sAMMS
Trauma Level/Designation: N/a

**Mileage Billed at the IRS Standard Rate

Requirement description : **Must notate all during name clears**
Board Certified (Addiction Medicine) - REQUIRED
Board Eligible (within 1 year of residency) - REQUIRED
Active KY license - REQUIRED
Provider Must be local - only mileage is reimbursed - REQUIRED
