FMG Medical Clinic in Bonney Lake, WA is looking for a LOCAL Family Medicine Primary Care Locum to cover Mon-Fri for 6 months- 1 year, beginning as soon as the provider can be credentialed. M-F ambulatory day clinic.


Assignment Duration(start/end):

  • ASAP - 1 year

    Schedule/Availability Requirements:

  • Est. days per month needing covered: M - F/weekly
  • Minimum number of shifts preferred/month (2 weeks/month etc): Full Time preferred
  • Minimum length of commitment required: 6 months
  • Are there any specific or urgent dates needing coverage? General coverage at this point
  • Multiple part time locums accepted to fill schedule? Yes, no more than 2. Full time is preferred
  • Shift Details (start/end times etc): 7:30a - 5:30p

  • Duties: In-person patient visits, task box, med refills, etc.

  • Air Travel: Local, preferred but not required. xx750/round trip cap
  • Rental Cars: xx65/day all in cap (including taxes and fees) regardless of car class.

Travel exception requests will be denied if this information is not part of the provider presentation.   • Malpractice - Must be included in rate.
  • Orientation - half the Call Back rate, kicks in after 8 hours. - \"Per hour of orientation or training beyond 8 hours\"

Account Managers and Account Coordinators will take this into account as they review the presentation, though they still reserve the right to redirect it back to the vendor if they deem it necessary.

  • Full legal name (first, middle, last)
  • Suffix for provider (MD, DO, PA, NP, CRNA)
  • NPI number must be entered in Trio
  • Provider email and best phone number (this pulls to the cover page for Client)
  • Best time to contact (this pulls to the cover page for the Client)
  • Years of experience must be updated in Trio
    • Years of locums experience is optional
  • License State where job is located... Copy of state license and the status. If provider s license in any other status besides active speak to Account Manager before presenting.
  • Other Active State License(s)- only list if not in good standing and provide explanation.
  • Certification (select multiple that apply; this pulls to the cover page for the Client)
  • Availability Section what is the providers ongoing availability (this pulls to the cover page for Client)
  • Answer the pertinent questions:
