Rate Caps for Submission:
Malpractice Must be included in rates.
Orientation half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly BR-BR60 - Per hour, not OT
Holiday Must be 1.5x the hourly rate. Per holiday hour, no OT
Nightly Call BR26-BR00 - Per night call to hold the pager, to include X hours gratis
Holiday Nightly Call - BR26-BR00 - "Per night call to hold the pager, to include X hours gratis
Call Back BR-BR60 - Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Holiday Call Back BR-BR60 - Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Schedule/Availability Requirements:
ASAP- as soon as credentialed - 6 months to a year
Assignment: The assignment is to work 5 days a week in the clinic setting or out-patient setting performing GI related procedures such as colonoscopy and endoscopy. Some call, the primary need is for clinic & procedures.

Duties: The clinician will provide consultative and follow-up care in the ambulatory clinic and also be scheduled to perform procedures 2-3 days per week.
Required Procedures: Standard GI colonoscopy and endoscopy
EMR: Cerner
Patients per Day: 16 to 20 in the clinic, 8-10 procedures per day in the out-patient center
Support Staff: assigned MA/procedure scheduler
Reason for Coverage: 1 of the 3 GI docs has tendered his resignation and we were already searching for 2 add l permanent physicians. There is a significant backlog of diagnostic and screening procedures.
Deal Breakers? Unwilling to provide all services requested.

Travel Preferences (see attachment for specific travel guidelines):
Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available, hotel/lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market.
