Backfilling provider coming up on their one year - candidates must have ERCP!
Rate Caps for Submission:
Malpractice Must be included in rates.
Orientation xx250 - half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Holiday Must be 1.5x the regular rates Per holiday hour, no OT
On Call (Daily) xx3600 and below - Per 24 hours to include 4 hours gratis"
24 Hour Holiday Call Rate - xx5400 - Per 24 hours to include 4 hours gratis"
Call Back xx500 and below - Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Holiday Call Back xx750 - Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Schedule/Availability Requirements:
24hr call
1-2 weeks/month

Requirement description :
**MUST NOTATE ALL DURING NAME CLEAR TO AVOID BEING REDIRECTED**
- Board Certified/Board Eligible REQUIRED
- Active CA license REQUIRED
- GI Fellowship REQUIRED
- Clean Malpractice strongly preferred
- Availability at time of name clear REQUIRED
- ACLS and DEA REQUIRED during credentialing
- ERCP REQUIRED
