Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

On Call (Daily) xx3,301-xx3,500 - Per 24 hours to include 4 hours gratis"

Holiday On Call (Daily) xx4,951.50 to xx5,250 - Per 24 hours to include 4 hours gratis"

Call Back xx376-$425 Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.

Holiday Call Back xx564 to xx637.50 Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.

Average worked Callback Hours: 5-7h

Schedule/Availability Requirements:

10/7 am- 10/12 am

10/31 am-11/5 am

5-6 days in December

Shift type- 24-hour call for OB and Gyn, No clinic or office patients.

Physicians do not need to remain in house but must be within 20-minute response time.

Facility Location: 2550 Sister Mary Columba Drive Red Bluff, CA 96080

Setting: Inpatient/Outpatient

Duties: Full scope OB GYN

FTE: 1.0

Required Procedures: Deliveries (C-section/vaginal), Gynecological surgeries

EMR: Cerner

Support Staff: Dedicated MA, Practice Manager

Reason for Coverage: permanent provider decreasing coverage

Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available, hotel/lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market.
