Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly BR-350 - Per hour, not OT

Holiday Must be 1.5x the regular rates Per holiday hour, no OT

Schedule/Availability Requirements:

• Start Date: January 2025
• Assignment: OB/GYN physician covering 1 week per month of clinic coverage, Mon-Fri only 9a-5p.

• Required Procedures/Job Duties: General OB/GYN procedures & job duties. Candidates will need to see patients in clinic,.
• EMR: Epic
  Support Staff: MA, and Sonographer in the clinic
• Reason for Coverage: Departure of existing physician and another physician ramping down in preparation for retirement.

EMR Required Training:

• Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
• EMR Virtual training Only able to attend if Modules are completed before training. Provider is required to schedule and complete their virtual EMR training prior to their first day, to avoid missing their first shift.

Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available, hotel/lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market.
