Summary: DHMF Half Moon Bay is looking for 1 Urgent Care physician starting ASAP through end of May. BC or truly BE in FM. Will consider BC or truly BE in IM if the candidate is comfortable seeing children. Details below.

Rate Caps for Submission:

• Malpractice Must be included in rates.
• Orientation half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
• Hourly xx190 and below - Per hour, not OT
• Holiday Must be 1.5x the regular rates Per holiday hour, no OT

Schedule/Availability Requirements: Wednesday Saturday, 9am-8pm with 1 hour lunch or dinner break. Please note shift times and dates- do not submit candidates that cannot commit to Wed-Saturday.

Duties: Urgent Care/Primary Care - All ages including children(no babies).

Required Procedures: Urgent Care related procedures

EMR: Cerner

Patients per Day: 15-25

Support Staff: 1 medical assistant and 1 front office staff

Reason for Coverage: Expansion with Urgent Care needs

Travel Preferences:

Credentialing Information:

Timeframe: 60-90 days

Requirements:

Credentialing with other facilities? DHMF and Sequoia hospital credentialing.

Billing enrollment: Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? No

• Vendors are now required to compile all presentation documents for a locum into a single PDF file.

• The PDF must be uploaded into TRIO in the "Candidate Documents" section under the locum's submission page.

• The PDF title should include the locum's name, site location, and specialty (ex. Dr. John Smith Good Samaritan Physician Nephrology)

• An up-to-date CV (solely) is still required to submit in Name Clear Documents initially when sending a new name clear. Vendors will have until bid due date to submit a PDF of ALL presentation documents. If PDF is not submitted, TRIO will not clear provider.

Required Documents for a Full Presentation Packet:

1. Up-to-date CV (Gaps greater than 30 days will need notation/explanation of gap in work history)

2. Malpractice documentation (NPDB and or write up). Must include ALL cases, dates, and payouts.

3. Disciplinary Actions (Must provide a detailed explanation)

4. Copy of Board Certification(s)

5. State License

6. Supporting certifications such as DEA, BLS, ACLS, ATLS, NRP, PALS

7. Copy of clean Sex Offender search
