Locum Physician Urology Durango Co Apply
Summary: Mercy Regional Medical Center is looking for a locum Urologist to help fill the dates detailed below. Candidates must be available for any of the stints below to be considered for ongoing dates. Site anticipates the need of 1 week per month ongoing beyond July 2025.

Rate Caps for Submission:
- Malpractice Must be included in rates.
- Orientation half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
- Hourly xx425 and below - Per hour, not OT
- Holiday Must be 1.5x the regular rates. Per holiday hour, no OT
- Nightly Call $950 and below - Per night call to hold the pager, to include 2.5 hours gratis
- Holiday Nightly Call - xx1,425 - "Per night call to hold the pager, to include 2.5 hours gratis
- On Call (Daily) xx3900 and below - Per 24 hours to include 4 hours gratis"
- 24 Hour Holiday Call Rate - xx5,850 - Per 24 hours to include 4 hours gratis"
- Call Back xx425 and below - Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
- Holiday Call Back xx637.50 - Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Average Call Back: 1-2 hours
Dates of Coverage:
- All shifts end the following day at 7a.
- Mar 31-Apr 6
- Apr 21-27
- Apr 28-May 4
- June 30-July 6
- Site will need about 1 week per month ongoing starting July 2025.
Details:
- 1 weeks per month ongoing
- Monday-Friday - 8a-4p + 4p-7a (night call) & Saturday-Sunday - 24hr call (7a-7a)
- Setting: Inpatient, Outpatient, Emergency Room, Clinic, Call Trauma Level III
- FTE: .5
- Required Procedures/Job Duties: Seeing patients in clinic
- OR time for various procedures
- EMR: EPIC
- Support Staff: MA, NP/PA, RN
- Reason for Coverage: Candidate submitted cancellation recently and team needs to backfill this provider.
EMR Required Training:
- Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
- EMR Onsite training Only able to attend if Modules are completed before training. Onsite EMR training will be schedule the morning of providers first day.
- All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission before an offer can be made:
- What are the # of years of epic experience they have?
- When was the last time (MM/YYYY) that they used Epic?
- What is the name of the last facility the provider used EPIC?
- Can you provide me with a photo large enough to add to a badge and not become pixelated?
- Can you disclose the provider's time zone for scheduling the virtual epic training?
- We need the provider's best phone # to use for call shifts while working. Can you disclose?
- We need the provider's best email, to use for programs while working onsite and the preapplication. Can you please send us their personal email address?
Travel Preferences:
- Preferred Lodging: Must follow CSH travel policy.
- Air Travel: Okay.
- Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information:
- Timeframe: 60-90 days

Requirement description :
- Board Certified Urology - Required, will not accept BE candidates
- Active CO License - Required or IMLC (LOQ in hand at time of submission)
- Active BLS - Required for credentialing
- Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
- Candidates must detail which of the stints open they are able to cover and must be available for any stint in job description to be considered for ongoing dates - Required
- Must list # of shifts per month ongoing the provider is able to offer to the team - Required
- Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required
- Clean Malpractice/background - Required, willing to accept 1 case up to xx300k - Candidates who present with a self inquiry NPDB report at time of presentation will be considered with priority over those that do not.
