Licensed Practical Nurse (LPN)Caregiver, Inc. is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide direct nursing care and support to an assigned caseload of consumers, including those receiving Wellness Coordination Services. The LPN works under the direction of an RN or Clinical Services Manager to ensure consumers' health needs are met in compliance with state regulations and agency policies.Key ResponsibilitiesProvide direct and individualized nursing care to assigned consumers in accordance with care plans and agency policies.Deliver Wellness Coordination Services as assigned, including required documentation and service provision per state regulations.Ensure prescribed medications and treatments are available in the home and administered as ordered.Monitor and document vital signs, weight, immunization records, and overall health status of consumers.Complete nursing assessments and documentation per agency policy and within LPN scope of practice.Maintain accurate, timely, and complete consumer medical records in compliance with HIPAA and applicable federal and state regulations.Report changes in consumer health status to the supervising RN or appropriate clinical leadership.Review medication administration practices and report medication errors according to agency procedures.Participate in Area Individual Support Team (IST) meetings and communicate relevant health information to team members.Maintain positive working relationships with consumers, families, guardians, and interdisciplinary team members.Serve as a point of contact during consumer hospitalizations, assisting with communication between hospital staff and agency personnel as directed.Assist with staff training related to health monitoring, medication administration, recognizing signs of illness, CPR, first aid, and other required competencies.Administer and read TB skin tests for consumers and staff as permitted by state regulations.Serve as a liaison with healthcare providers and allied service professionals as assigned.Participate in the nursing on-call rotation as scheduled.Attend routine supervisory reviews with the Clinical Services Manager or supervising RN.Designate a qualified individual to act on their behalf during absences, as appropriate.Perform other duties as assigned.QualificationsCurrent Licensed Practical Nurse (LPN) license in good standingKnowledge of basic nursing care, medication administration, and documentation standardsAbility to work collaboratively with RNs, direct support staff, families, and healthcare providersExperience working with individuals with intellectual or developmental disabilities preferredBenefitsNO CREDIT CHECK bank accounts to all employees with cash advance opportunities through Branch Bank.$10,000 in tuition reimbursement annually to further your education endeavors for full-time eligible employees.$10,000 in life insurance for all employees at no costMedical, Dental, Vision, Voluntary Life and AD&D, Supplemental Insurance, and 401K for eligible employees.
