Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment.
Compensation & Benefits
\tBase Pay: $35/hour
\tOpportunity to earn up to $43/hour with applicable differentials:
\t
\t\tTrach: +$2/hour
\t\tVent: +$3/hour
\t\tNights (7pm–7am): +$2/hour
\t\tWeekends: +$3/hour
\t\tHolidays: +$3/hour
\tWeekly pay

\tCases available in close proximity to your home

Key Responsibilities
\tDeliver skilled nursing care, including G-tubes, tracheostomies, ventilators, and central lines
\tMonitor patient condition and respond appropriately to changes
\tEducate patients and families on care plans and procedures
\tCollaborate with interdisciplinary team members to ensure optimal outcomes
QualificationsQualifications
\tActive LPN license in Georgia or Nurse Licensure Compact
\tCurrent Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization is required.
\tA minimum of one year of LPN experience required.
\tValid PPD skin test
Physical Requirements of the Role
\tAbility to stand or sit for extended periods (up to 12 hours)
\tAbility to bend, twist, kneel, and assist with patient mobility
\tAbility to lift, push, or pull up to 50 pounds or more
\tManual dexterity for clinical tasks
