Key Responsibilities
- Administer medications and treatments as prescribed
- Monitor residents’ vital signs and overall health status
- Assess and document changes in condition
- Provide wound care and other skilled nursing procedures
- Assist residents with activities of daily living (ADLs) when needed
- Develop, implement, and update care plans in collaboration with the healthcare team
- Supervise Certified Nursing Assistants (CNAs) and delegate tasks appropriately
- Communicate with physicians, families, and interdisciplinary team members
- Maintain accurate and timely documentation in medical records
- Ensure compliance with state and federal regulations
Requirements
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing
- Graduate of an accredited practical nursing program
- CPR/BLS certification
- Knowledge of geriatric care and chronic disease management
- Strong clinical assessment and critical thinking skill
