Key Responsibilities

  • Administer medications and treatments as prescribed
  • Monitor residents’ vital signs and overall health status
  • Assess and document changes in condition
  • Provide wound care and other skilled nursing procedures
  • Assist residents with activities of daily living (ADLs) when needed
  • Develop, implement, and update care plans in collaboration with the healthcare team
  • Supervise Certified Nursing Assistants (CNAs) and delegate tasks appropriately
  • Communicate with physicians, families, and interdisciplinary team members
  • Maintain accurate and timely documentation in medical records
  • Ensure compliance with state and federal regulations

Requirements

Required Qualifications

  • Current Licensed Practical Nurse (LPN) license in good standing
  • Graduate of an accredited practical nursing program
  • CPR/BLS certification
  • Knowledge of geriatric care and chronic disease management
  • Strong clinical assessment and critical thinking skill
