We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at St Johnsbury Center for Living and Rehabilitation in Saint Johnsbury, Vermont. As a vital member of our interdisciplinary care team, you will provide high-quality patient care to our residents, delivering exceptional service with a focus on comfort, dignity, and respect.

Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living

• Develop and implement individualized care plans, in collaboration with our interdisciplinary team

• Monitor and report changes in patient condition, concerns, and needs to the healthcare team

• Maintain accurate and complete records of patient care, medications, and treatments

• Participate in quality improvement initiatives to optimize patient outcomes and satisfaction

• Collaborate with other healthcare professionals to provide comprehensive care

• Demonstrate a commitment to delivering exceptional customer service, promoting a positive and supportive environment for our residents and families

Requirements:

• Current LPN licensure in the state of Vermont

• Strong communication and interpersonal skills, with the ability to work effectively with diverse personalities and needs

• Ability to work varied shifts, including overnight hours, as needed

• Commitment to maintaining a professional, respectful, and compassionate attitude in all interactions
