Position Summary

Seeking an experienced Licensed Practical Nurse (LPN) for a Long-Term Care (LTC) setting within a small rural critical access hospital environment in Hawaii. The LPN will provide direct patient care to residents requiring extended nursing support and assist in maintaining a safe, therapeutic care environment.

Travelers must obtain an active Hawaii LPN license upon offer acceptance and hold the license prior to the start of the assignment.

Facility Overview
  • 21-bed critical access hospital

  • On-site rural health clinic providing family practice services

  • Small, community-focused healthcare setting

Key Responsibilities
  • Provide direct nursing care to long-term care residents

  • Administer medications and treatments as prescribed

  • Monitor and document patient status in EPIC EMR

  • Assist with activities of daily living (ADLs)

  • Communicate patient condition changes to supervising RN and provider

  • Support care planning and interdisciplinary collaboration

  • Maintain compliance with infection control and safety standards

Schedule
  • 13-week assignment

  • 12-hour shifts

  • 6 shifts per 2-week pay period

  • Variable scheduling

  • Holiday coverage required

Requirements
  • Active LPN license (Hawaii license required prior to start)

  • Minimum two (2) years of LTC experience preferred

  • Experience working in long-term care setting required

  • Current BLS/BCLS certification

  • Ability to work independently in a rural healthcare setting

Additional Information
  • EPIC EMR experience preferred

  • Candidate must arrive prior to assignment start to complete employee health clearance

  • Flexibility required in a small facility environment
