Job Summary
We are seeking a skilled and motivated Home Health LVN to join our team. The ideal candidate will provide high-quality, patient-centered care, supporting home-based health management for individuals in need of skilled nursing services. This role requires strong clinical skills, attention to detail, and a commitment to patient comfort and well-being.

Duties

• Conduct home visits to provide direct care, including wound care, medication administration, and monitoring vital signs.
• Develop and implement individualized care plans based on patient assessments.
• Educate patients and families on home care procedures and chronic illness management.
• Document all procedures accurately in electronic health record systems, such as Epic or Meditech.
• Communicate effectively with physicians, case managers, and other healthcare team members to ensure comprehensive patient care.
• Follow infection control protocols and maintain patient safety standards.

Experience

• Minimum 2 years of experience as an LVN in a home health or high-acuity care setting.
• Experience in home health or community-based care settings is preferred.

Requirements

• Active California LVN license.
• Certification in BLS (Basic Life Support) is required.
