Major Duties And Responsibilities Provide direct nursing care to residents, including treatments, under the medical direction of the resident’s attending physician. Supervise the day‑to‑day nursing activities of nursing assistants in accordance with federal, state, and local standards, guidelines, and regulations, and as required by the Director of Nursing Services or Nurse Supervisor to ensure the highest quality of care. Ensure that all nursing personnel comply with written policies and procedures established by the facility. Meet with assigned nursing staff and support personnel to plan shift services, programs, and activities. Participate in facility surveys conducted by authorized government agencies when requested. Handle admissions, transfers, and discharges of residents as required. Complete accident/incident reports as necessary. Maintain the daily report. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, and other necessary documentation. Receive telephone orders from physicians and record them as per facility policy. Administer medications as ordered by the physician in accordance with facility policy and procedure. Maintain accurate controlled drug records for your shift and count controlled drugs with the incoming shift. Participate in orientation of new employees to your shift, policies, and procedures applicable to their job positions. Attend and participate in in‑service education programs. Monitor nursing personnel to ensure safety standards and infection control policies and procedures are followed. Maintain the confidentiality of all resident care information. Know and comply with regulations ensuring that all personnel respect residents’ rights. Physical and Sensory Requirements Able to communicate with residents and personnel. Able to remain calm under stress. Able to read and write English. Implementation of Basic Nursing Care Assess basic physical, emotional, and social needs of residents. Collect data for admissions assessment. Develop and update residents’ comprehensive care plans. Assist residents with activities of daily living and encourage appropriate self‑care. Safely perform therapeutic nursing techniques and skills as per clinical assessment. Provide nursing care in conformance with established standards and policies and in keeping with the medical regimen. Maintain accurate and complete records of observation of residents, nursing care provided, and the resident’s response to care. Teach residents and significant others good health habits and document accordingly. Administer medications safely. Transcribe doctor’s orders accurately on the Medication Administration Record (MAR). Administer medication accurately. Chart medications accurately. Make significant resident observations and communicate these to the healthcare team. Maintain resident confidentiality. Support the resident in their expression of quality of life, dignity, and rights. Charge Nurse Responsibilities Manage the care of residents on the unit. Recognize and respond to changes in residents’ conditions in a timely manner. Communicate with the physician regarding changes in residents’ condition and accept telephone medical orders. Maintain standards for compliance with NYS DOH LTC survey and participate in the survey process. Participate in the interdisciplinary team resident assessment meetings. Complete incident reports and contribute to investigations as necessary. Assign personnel and direct the day‑to‑day unit function. Participate in the documentation of monthly summaries and other documentation specific to long‑term care. Report unit and resident conditions and seek guidance from the nurse manager and supervisor. Qualifications Completed a practical nursing program approved by the New York State Education Department and hold a valid New York State LPN license. Current, unencumbered, active license to practice as an LPN in New York State. Ability to make independent decisions when circumstances warrant such action. Knowledge of nursing and medical practices and procedures, and laws, regulations, and guidelines pertaining to long‑term care. Leadership and supervisory ability and willingness to work harmoniously with and supervise other personnel. Previous experience in a skilled setting is a plus.
