The LPN is responsible for providing nursing care consistent with the goals and philosophy of the Wythe Physician Practices. The LPN is required to provide professional nursing care and assists in the delivery of health care management for patients. This position requires strong clinical and critical thinking skills. The LPN must be able to multi-task in a very fast paced, family practice/internal medical office environment. May be required to help cover other areas as needed in all Physician Practices clinics. Responsibilities Assist the healthcare provider in all Wythe Physician Practice Clinics. Provide direct patient care: equipment preparation; obtaining and updating patient history, vital signs, medication lists; scheduling appointments and procedures as ordered by provider; responsible for prescription refills as authorized; obtaining insurance pre-authorizations for testing and procedures. Maintain a clean and safe environment following practice policies. Stay competent in all Wythe Physician Practice Clinics. Administer prescribed oral, inhalation, intramuscular and subcutaneous injections per provider orders and practice policy. Complete safety instructional programs and enforce accepted safety practices for patients, clinic, and hospital. Report all incidents, hazards, drug interactions and initiate appropriate action. Complete assigned job duties within appropriate working hours and manage time wisely. Technical Requirements Knowledge of ICD-10, CPT codes, insurance benefits, and healthcare backgrounds. Proficiency with all technical/medical equipment in all clinics. Proficiency in EMR, order entry and other clinic and hospital systems. Working knowledge of computers, telephones, copy and fax machines. ECW and Meditech experience preferred. Supervision Must work independently with minimal supervision and prioritize and organize work areas. Patients Served 13–18 years (Adolescent), 19–64 years (Adult), 65+ years (Geriatric). Working Conditions & Hazards Exposure possibilities: None, Some, Frequent, Very Frequent. Potential hazards: chemicals; blood or body fluids; extreme conditions; communicable diseases; dust/fumes/gases/odors (including asbestos); unprotected heights; moving mechanical parts; CRT monitor; potential electrical shock; frequent repetitive motions; X‑ray electromagnetic energy; abuse or combative patient; high‑pitched noises. Functional Requirements Perform medium to heavy work: frequent lifting of up to 50 pounds; wet/slippery conditions; stairs; moving beds or furniture; stooping, kneeling, reaching and grasping. Verbal communication and auditory perception. Knowledge of infection control standards, isolation room techniques and policy. Personal Protective Equipment This position requires the use of personal protective equipment. Qualifications Education: Graduate of an accredited school of Licensed Practical Nursing.
