Shift Notes: 8hr days 40hr guarantee
Weekend Requirements: Rotating Weekends/Holiday Coverage Required if needed
On-Call Requirements: No Call

** AT LEAST TWO SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW MUST BE ADDED TO THE PROFILE - PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION. "ANYTIME" WILL NOT BE ACCEPTED.
THIS IS A HOME HEALTH NEED!
The physical therapist is responsible for providing physical therapy services to clients of the agency. The primary function is the provision of direct patient care, including collaboration with other team members and participation in the development of the patient's comprehensive plan of care.
Rehabilitation equipment including but not limited to:
- splints,
- orthoses
- prostheses,
- wheelchairs,
- ADL equipment
- Modalities.
- Candidate required to be Home Health Certified
- 1.5+ acute nursing care (hospital-based) and Home Care experience
- A valid Driver's License is required for this need
- Must be able to work independently with minimal supervision/assistance
- Acute care experience preferred
- Certified home health experience including proficiency with OASIS required and NON-NEGOTIABLE
- Work History
- Skills Checklist (pertinent to position), current to 1 year
- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications - Must be uploaded with profile
- BLS (AHA)
