Job Title: MDFT Therapist
Location:Hempstead, NY
Job Description:
Position Summary:
The MDFT Therapist provides intensive, home- and community-based clinical services to youth and their families. The therapist maintains a caseload of up to 10 clients and delivers services in alignment with the MDFT model to promote behavioral change, strengthen family functioning, and reduce justice system involvement.
Key Responsibilities:
  • Develop comprehensive clinical treatment plans based on assessments and intake evaluations.
  • Provide individual and family therapy utilizing the MDFT model.
  • Deliver trauma-informed care in home, community, and office-based settings.
  • Participate in interdisciplinary team meetings and case reviews.
  • Attend required trainings and ongoing professional development in MDFT and other evidence-based practices.
  • Complete documentation and electronic medical records accurately and in a timely manner.
  • Perform additional duties as required to meet client, agency, and regulatory standards.
Minimum Qualifications:
  • LMSW, LCSW, LMHC, or LMFT (Required).
  • Minimum of 1 year experience working with children and/or adolescents.
  • Bilingual in English and Spanish (Required).
  • Valid NYS Driver's License in good standing and access to a reliable vehicle.
  • Proficiency in electronic health records and Microsoft Office.
Physical Requirements:
  • Ability to read and write reports and case documentation.
  • Ability to walk, stand, and sit for extended periods.
  • Ability to bend, stoop, kneel, squat, climb stairs, and reach overhead.
  • Ability to perform repetitive wrist and finger movements.
