The State of New Hampshire, Department of Health and Human Services, and New Hampshire Hospital has a full-time vacancy for Medical Records Technician.

Summary:
Performs all functions related to release of protected health information (PHI) including maintaining integrity of the Release of Information database. Analyzes medical records for completeness after discharge, posts filing to medical records. Transcribes medical record reports. Utilizes an electronic and paper (hybrid) medical record environment. Scans medical record documents to the electronic health record. Works under the direct supervision of the Administrator of the Health Information Department.

MINIMUM QUALIFICATIONS:

Education: Associate's degree (two years of college or vocational/technical school) with a major study in Health Information Management or a related health care field. Each additional year of approved formal education may be substituted for one year of required work experience.

Experience: Three years' experience involving the maintenance of medical records as a medical clerk or in an area which provided experience with medical terminology and records. Each additional year of approved work experience may be substituted for one year of required formal education.

License/Certification: Registration as a RHIT (Registered Health Information Technician) or Registered RHIA (Registered Health Information Administrator) issued by the American Health Information Management Association (AHIMA) is preferred.

Special Requirements: Standing or sitting for prolonged periods of time. Climbing ladders with 3-4 steps. Hearing acuity to answer telephones and transcribe dictation. May require lifting boxes weighing up to 20 lbs. Reaching above one's head to retrieve files or boxes. Visual acuity to accurately file medical record information and view information on computer screens. Manual dexterity to use computer keyboard, sort and file medical information.

SPECIAL QUALIFICIATIONS :

Computer skills utilizing a variety of software including word processing, spreadsheet and database software, and Windows operating system are required.

Ability to pass Medical Terminology test with a minimum score of 80% accuracy is required

Ability to pass a typing test at a speed of 65+ words per minute with no more than three errors is required.

Ability to demonstrate, through prior responsibilities, a high level of attention to detail is required.

Ability to work with a variety of customers, patients, attorneys and other healthcare providers in a diplomatic and professional fashion is required

Knowledge of HIPAA, State, Federal guidelines for release of protected health information is desirable.

Knowledge of Joint Commission standards related to medical record content and timeliness of completion is desirable.

Responsibilities :

  • Collects, maintains and releases protected health information in accordance with HIPAA, Federal, State regulations and hospital policies on the release of patient information. Responsibilities include date stamping all incoming correspondence. Logging in all requests for PHI in the Release of Information database and updating when request is completed. Prior to sending any information the authorization to release information is verified for accuracy, appropriateness, completeness and compliance with HIPAA guidelines.
  • Performs all health information responsibilities including transcription, patient record maintenance, and the release of protected health information at the New Hampshire Hospital HID in accordance with established Hospital policies and procedures.
  • Protected health information is sent to continuing care providers in accordance with timeliness standards as established by Federal, State or as specified in managed care contracts.
  • Assists in the development of operational policies in maintaining an incomplete Record Tracking procedure to facilitate the completion of discharged patient records within 8 days of discharge. Notifies the clinical staff on a weekly basis or more often if needed of their incomplete patient records.
  • Processes inpatient admissions, discharges in accordance with Joint Commission and other regulatory standards to maintain reliable statistical data. Verifies completion of deficiencies by clinical staff utilizing an electronic database
  • Transcribes technical medical information and assessments such as psychological and psychiatric reports, special educational assessments, nursing reports and discharge progress notes in a timely and accurate manner to compile and provide specialized reports on patient information to the follow-up care community and to meet Joint Commission accreditation standards pertaining to documentation requirements. Transcribes multidisciplinary discharge summary to accurately communicate information needed to facilitate follow-up care and future placement of an in-patient.
  • Manages the electronic and paper medical record at the time of discharge in a manner consistent with Joint Commission, Federal, State and other regulatory requirements. This includes but is not limited to continuous quality management by verification that all documents in the paper and electronic record are filed/scanned in the correct record. Notifies supervisor of any documents that need to be moved from the electronic record of one patient to another.
  • Assumes responsibility for the program maintenance of an accurate chart location system to promote timely and efficient retrieval of medical records to meet patient care needs, accrediting agency standards and state and federal regulations.
  • Assists with operational training of the Unit Secretary and staff members of New Hampshire Hospital medical record policies and procedures. Joint Commission requirements, state and federal regulations related to the content, maintenance of medical record documentation and release of protected health information. Presents information to clarify policies, procedures and standards.
  • Enters work orders in the electronic work order system for equipment, structural or other repairs that may present a staff or patient safety issue.
  • Must have the ability to work in a noisy environment with many interruptions and have the ability to maintain highly confidential patient specific protected health information.

  • Performing accountabilities in accordance with established policies and procedures.
  • Communicating with supervisors' issues related to workplace safety (i.e., ergonomics, body mechanics, environment, equipment, etc.).

  • Attending training programs, as appropriate.
