We are committed to supporting families every step of the way during this intimate journey. Role Description This is a part-time hybrid role for a Medical Social Worker based in Sacramento, CA. The primary responsibilities include providing emotional and psychosocial support to patients and families, conducting assessments, developing care plans, facilitating discharge planning, and coordinating community resources. The role requires collaboration with the interdisciplinary care team to address patients' unique needs and deliver comprehensive care. Qualifications Proficiency in Medical Social Work and Social Work practices Experience in Discharge Planning and Case Management Knowledge of and ability to navigate Social Services systems Strong interpersonal, communication, and cultural competency skills Active State of California license or certification in social work (LCSW, MSW preferred) Ability to work both independently and as part of an interdisciplinary team Prior experience in hospice or palliative care is a plus Driver’s license and access to reliable transportation for local travel #J-18808-Ljbffr
