Holly Patterson Extended Care Facility is our 589-bed skilled nursing facility, which maintains a 5-star rating in quality measures. We are nationally recognized as a model offering innovative care in an environment that treats the whole person. Our skilled and caring medical staff responds to the physical, social and emotional needs of each resident. Currently, were experiencing some exciting changes, including expanding our services and developing new programs.

We currently have exceptional opportunities for a full-time Social Worker at A. Holly Patterson Extended Care Facility. Responsibilities include interviewing patients and preparing social histories. Candidates must be able to maintain case records, prepare reports and determine techniques and community resources to use in planning rehabilitative programs for patients.

We require:

Continuing possession of a current license as a Master Social Worker (LMSW) or a Clinical Social Worker (LCSW), issued by the New York State Education Department.

NOTE: In lieu of the above, possession of a limited permit to practice at the Nassau Health Care Corporation is acceptable.
