ABOUT THIS JOB: At New Horizons, we are committed to providing exceptional services and support to individuals with Intellectual and Developmental Disabilities (IDD). We are currently seeking a passionate and dedicated Member Services Coordinator to join our team. In this role, you will be responsible for coordinating services and programs that enhance the quality of life for our Members.

 

JOB DESCRIPTION: As a Member Services Coordinator, you will play a vital role in ensuring that our Members receive the support and resources they need. You will work closely with our Members, their families, and our team of professionals to assess needs and develop individualized plans. Additionally, you will coordinate the implementation of services and monitor progress to ensure that goals are being met. This includes coordinating with external service providers, advocating for Members, and providing ongoing support and guidance.

Pay Rate: $21.50-23.00

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Demonstrates positive attitude and professional communication with all stakeholders.
  • Enters and updates accurate information in the Member database.
  • Ensures Members have proper authorization and accurate attendance tracking.
  • Assists in the development and implementation of procedures, recordkeeping, schedules, and training.
  • Works collaboratively with MSCs to provide training to DSPs.
  • Participates in the intake process and completes/ reviews Special Incidents Reports.
  • Engages in problem-solving and devises effective solutions as needed.
  • Coordinates Members' transportation and ensures DSPs are trained.
  • Completes and submits all paperwork and documentation accurately and timely.
  • Contacts/engages with assigned Members and participates in meetings with other MSCs and supervisors.
  • Participates in the development and progress review of Members' plans and curriculum.
  • Communicates with Members' Regional Center Services Coordinators.
  • Participates in trainings and serves as a back-up to other MSCs as needed.
  • Assists Members with toileting accidents as needed.

 

Requirements

  • A high school diploma or equivalent is required; associate’s degree is a plus; two or more years of experience working with one of the following: adults/children with developmental/intellectual disabilities, elderly, individuals with mental health disorders.
  • Prior experience in providing services and support to individuals with IDD
  • Strong organizational and time management skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Proficient in Microsoft Office Suite
  • Valid driver's license and access to reliable transportation
  • Authorized to work in the United States
