Shelter House was established in 1981 as a grassroots response to the homelessness crisis in Fairfax County. Since then, we have grown and adapted to meet evolving community needs through partnerships with government agencies, private partners, and local residents.

Today, Shelter House is a reputable nonprofit with more than 100 employees committed to preventing and ending homelessness and domestic violence. In the last year, our team supported nearly 2,000 individuals, half of whom were children. About the Role

The Mondloch Place Case Manager supports a caseload of 20 efficiency style units in the Permanent Supportive Housing Program. Program participants often face complex challenges such as mental health diagnoses, substance use, developmental or physical disabilities, limited income, and chronic homelessness.

Using a Housing First model, trauma-informed care, and harm reduction, the Case Manager helps clients maintain housing stability, build self sufficiency, and progress toward independence. Responsibilities include assessments, case planning, resource linkage, crisis support, and coordination with internal and external partners.

How You Will Contribute

Duties and Responsibilities

  • Use a strengths based approach to assess client strengths, physical and mental health, social needs, education, life skills, and employment readiness.
  • Partner with clients to develop individualized service plans and identify supports needed to achieve stability and long term goals.
  • Support clients in engaging in social and community activities and help remove barriers to participation.
  • Assist clients in securing needed services such as employment, housing, financial assistance, mental health services, SSI applications, and more.
  • Provide crisis intervention, implement emergency procedures when needed, and teach coping strategies.
  • Create and maintain budgets with clients and provide money management education.
  • Support clients in ensuring rent and utilities are paid on time.
  • Maintain complete and accurate client records following agency and best practice standards.
  • Document all client interactions in client files and HMIS and complete annual reassessments and quarterly reviews.
  • Participate in staff meetings and trainings as required.
  • Ensure compliance with all agency policies.
  • Work collaboratively as part of a multidisciplinary team within Shelter House and with partner agencies.
  • Share relevant client information with appropriate staff in a timely manner.
  • Use Microsoft Office Suite applications as part of daily work.
  • Safely operate Shelter House vehicles to transport clients when necessary.
  • Demonstrate the Shelter House core values: Inclusivity, Collaboration, Accountability, Respect, and Empowerment.
  • Perform other duties as assigned.

About You

Required Qualifications

  • Bachelor's degree in human services or a related field, or equivalent experience.
  • Two or more years of case management, direct services, or related experience.
  • Strong written and verbal communication skills.
  • Ability to prioritize tasks, manage competing deadlines, and exercise sound judgment.
  • Ability to complete work despite interruptions and shifting needs.
  • Creative and resourceful approach to problem solving.
  • Strong organizational and time management skills.
  • Valid Driver’s License and reliable transportation.
  • Ability to drive a 15 passenger van.
  • Willingness to work evenings or weekends as needed.

Preferred Qualifications

  • Fluency in a second language such as Spanish, Farsi, Arabic, or Amharic.
  • Experience working with clients experiencing homelessness, mental health challenges, or substance use disorders.
  • Experience using HMIS.

Physical Requirements

  • Annual TB test.
  • Ability to sit or stand for extended periods.
  • Ability to lift 10 to 20 pounds.
