Shift Type/Shift Times/Hours/Day: Nights, 6:00 PM - 6:30 AM, 12

Shift Notes: 6:00 PM - 6:30 AM/36 hours schedule/24 hours guaranteed/client is allowed to cancel 1 shift per week/Must be willing to float to units where experienced

Weekend Requirements: Weekend schedule to be determined by facility

On-Call Requirements: On-call schedule to be determined by facility

• Learning modules must be completed before the start of the assignment.
• Non-billable hours have been adjusted to reflect 16 hours for each assignment.
• Client or Vendor may cancel an Assignment in progress with 7 days written notice without penalty.

** Client has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in. This helps us get candidates presented quicker as we all know these orders open/close quickly!

** This form needs to be 100% accurate and have all requests noted in the highlights section.

• Start Date should be on or very quickly after the Order Start Date listed.
• All RTO should be listed on the cover page and must be less than 7 days.
• RTO will not be approved in the first 2 weeks of the assignment.
• All facilities the traveler is open to working in should be listed on the submission.
• If interested in "any" location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
• All shifts the traveler is open to working on should be listed on the submission.
• All departments the traveler is open to working must be checked on the cover page.
• All candidates submitted should be preclosed and ready to accept.
• Please let us know if the traveler withdraws or is no longer available.

SUBMISSION REQUIREMENTS

• Minimum 24 months of current Labor and Delivery experience - REQUIRED
• Must be willing to float to Med Surg unit - REQUIRED
• Certifications must be American Heart Association OR American Red Cross and active upon submission - MUST BE INCLUDED IN SUBMISSION - REQUIRED
• No Nursy Hits - REQUIRED
• Travel experience - REQUIRED

• Resume with all facilities, facility size, beds, trauma level, and units listed
• Skills Checklist specific to the discipline (must be less than a year old)
• Active License (verification must be less than 30 days old)
• Certifications (must be active at the time of submission)
• Driver's License
