Ascendant Health leads with innovative ideas, patient choice, great provider network, and promoting awareness of traumatic brain injury effects.Role DescriptionThis is a full-time hybrid Nurse Case Manager role located in Grand Rapids area, with some flexibility for remote work. The Nurse Case Manager will coordinate all aspects of injury-related care, advocate for their patients, and develop a care plan to manage care complexities.QualificationsStrong communication and interpersonal skillsExcellent organizational and time-management abilitiesAbility to work independently and as part of a teamKnowledge of healthcare regulations and case management processesWe can provide training and do pay for training as wellBenefitsCompetitive pay10 days of PTO earned per year if full time that increases to 15 days after two yearsPaid trainingHealth insurance coverage (we cover this in full for our employees typically)Easily earn your hours, you control your scheduleLiberal pay for each case management activityHybrid role so you get the best of both worlds (about 50/50)Amazing team members that will help you on an ongoing basis with questions or issuesRoom to grow and become a senior case managerFun happy hours out at the hottest restaurants and rooftops with the team
