Job Description:

• Conduct 10 12 patient visits daily across 3 8 DaVita dialysis centres in the Chesapeake/Norfolk area.

• Perform comprehensive health evaluations and manage chronic conditions such as diabetes, hypertension, COPD, and heart disease.

• Prescribe, titrate, and monitor medications for complex chronic conditions.

• Coordinate patient care with nephrologists, dialysis teammates, and primary care providers.

• Lead care innovation, providing feedback to DaVita's leadership team on new models of care.
