Nurse Practitioner / Certified Physician Assistant

Battle Creek / Kalamazoo Cluster, MI


Key Responsibilities:

  • Conduct routine weekday rounding in assigned skilled nursing facilities
  • Perform focused patient assessments (“micro visits”) to evaluate recovery progress, pain, wounds, and functional status
  • Develop and implement individualized care plans in collaboration with physicians, therapists, and facility staff
  • Complete accurate and timely documentation using EHR systems (remote flexibility available)
  • Identify and manage readmission risks, ensuring continuity of care
  • Maintain compliance with post-acute care standards and PM&R-related regulations


Qualifications:

  • Master’s degree in Nursing (NP) or completion of an accredited Physician Assistant (PA) program
  • Active, unrestricted NP or PA license with prescriptive authority in the state of practice
  • Minimum 2–3 years of experience in geriatrics, post-acute care, PM&R, or related field (nursing facility experience preferred)
  • Strong clinical skills in managing recovery barriers, chronic conditions, and rehabilitation needs
  • Excellent communication and time-management skills
  • Proficiency with EHR systems and remote documentation
  • Ability to work independently with strong clinical judgment and consistency
