Overview Matrix Medical Network is a national in‑home clinical network delivering comprehensive health assessments and care management services. Responsibilities Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, referrals, case management and documentation. Administer and order point‑of‑care and lab‑based testing as needed. Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel. Work with the Primary Care Physician on patient education and follow‑up. Provide services at the member’s place of residence (home or skilled nursing facility). Benefits Flexibility: Visits are scheduled based on your availability (Sunday – Saturday, 8 am‑7 pm). Competitive pay: $90‑$115 per assessment; monthly $1,800 to $11,500 depending on visits (20‑100 / month). Sign‑on bonus for full‑time transition. Mileage reimbursement, 401(k), employee referral bonuses, voluntary benefits. Comprehensive onboarding, preceptor, 1:1 feedback, free continuing education. Support for additional state licensure and credentialing. Qualifications Master’s degree or equivalent, satisfactory completion of NP licensure. Current NP licensure in state of practice including prescription authority, or capability to obtain prescriptive authority. AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care. Current BLS, ACLS or CPR certification. 1 year NP experience preferred; new graduates encouraged. Valid state driver’s license and automobile insurance (state requirements). Liability coverage $100,000 per person/$300,000 per incident; property damage $50,000; collision/deductible $1,000 or less. Strong computer skills and familiarity with employee health/medical record software. Excellent verbal and written communication skills.
