Overview:
Wound Management Specialists, LLC is seeking a Mobile Wound Care Practitioner to join its Clinical Team. The Wound Care Provider works collaboratively with the team to deliver wound care to patients within skilled nursing, long-term care, transitional care, assisted living, memory care, private homes with or without home health. This position is responsible for providing wound management in collaboration with the facility wound nurse/nursing team, ensuring successful wound care outcomes. The ideal candidate will have the ability to interpret clearly and establish/maintain effective working relationships with patients, medical staff, and facilities.
Responsibilities:
- Conduct in-home advanced wound care visits for patients in the Lafayette area.
- Collaborate with interdisciplinary healthcare teams to provide wound care in various settings, including skilled nursing, long-term care, transitional care, assisted living, memory care, and private homes.
- Work closely with the facility wound nurse/nursing team to create and maintain effective wound care plans.
- Perform assessments, wound measurements, and provide evidence-based treatment modalities to optimize patient outcomes.
- Educate patients and their families on proper wound care techniques, preventive measures, and self-management strategies.
- Utilize cutting-edge wound care technologies and stay current with industry best practices through continuous education.
- Maintain accurate and timely documentation of patient assessments, interventions, and outcomes.
Qualifications:
- Louisiana APRN Nurse Practitioner License (Required) or Physician Assistant License (Required)
- Previous wound care experience is highly preferred, but not required. Wound care training and certifications will be provided upon hiring.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Work well independently in addition to being skilled in interdisciplinary collaboration.
- Is a self-starter, motivated, and demonstrates a positive attitude.
- Ability to interpret clearly and establish/maintain effective working relationships with patients, medical staff, and facilities.
- Willingness to travel up to 75% within the designated coverage area.
