This role offers the opportunity to work independently, manage complex medical conditions, and build strong, trusted clinical relationships all while being supported by a collaborative interdisciplinary team.You will serve as a consistent clinical presence within your assigned facilities, providing proactive, patient-centered care and responding to urgent medical needs to ensure continuity, quality, and dignity in treatment.Roles and Responsibilities Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings. Independently evaluate, diagnose, and treat acute and chronic medical conditions. Conduct initial admissions and follow-up assessments, including physical exams, medication review, and clinically appropriate screenings. Monitor changes in resident condition and adjust treatment plans accordingly. Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice. Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams. Communicate clearly and compassionately with residents and families regarding diagnoses, treatment plans, and changes in condition. Maintain accurate and timely documentation in the electronic medical record in accordance with regulatory and payer standards. Participate in quality and care coordination efforts to support excellent patient outcomes. Adhere to HIPAA, state, and federal regulations while upholding ethical standards of clinical practice.Minimum Qualifications Active, unrestricted Nurse Practitioner license in the applicable state(s) of practice. Current national Nurse Practitioner certification (ANCC, AANP, or equivalent). Clinical training and experience in adult, family, and/or geriatric medicine. Eligibility to work in the United States.Preferred Experience Experience in internal medicine, geriatrics, cardiology, or post-acute care. Active DEA registration or ability to obtain. Strong clinical judgment and ability to work independently. Excellent communication and interpersonal skills. Organized, detail-oriented, and able to manage multiple priorities effectively. Prior experience in SNF, LTC, ICU, or Emergency Department settings is a plus.
