JOB DESCRIPTION:

AdvisaCare, a leading home healthcare provider since 1997, is seeking a Home Health Occupational Therapist on a PRN basis! Our clinicians offer personalized one-on-one care to patients and families in their homes through teamwork and collaboration. As a Home Health Occupational Therapist, you will help patients regain their independence and maintain safety with their daily living activities through techniques such as therapeutic exercises and activities, adaptive equipment, and home modifications. You will work alongside a team of clinicians and healthcare staff to provide high-quality care to patients in your local community.

Responsibilities

  • Evaluate patients' functional abilities and limitations, including their home environment, to determine their needs and goals for therapy
  • Develop individualized treatment plans to improve patients' physical, cognitive, and psychosocial function, and track their progress towards goals
  • Utilize therapeutic exercises and activities, adaptive equipment, and home modifications to help patients regain or maintain their independence in daily living activities
  • Educate patients and caregivers on how to safely perform daily living activities and use specialized equipment
  • Communicate with physicians and other healthcare professionals to coordinate care plans, and make referrals for other services as needed
  • Maintain accurate and up-to-date documentation to meet regulatory and insurance requirements
  • Participate in team meetings to optimize patient outcomes

Requirements

  • Current license to practice as an Occupational Therapist in the state of Michigan
  • Degree in Occupational Therapy from an accredited program
  • Minimum of 1 year of experience in home health care is preferred
  • Strong communication and interpersonal skills to collaborate with patients, caregivers, and other healthcare professionals
  • Ability to work independently and manage a caseload efficiently
  • Basic computer skills to document patient care accurately
  • Reliable transportation and a valid driver's license, as travel to patients' homes is required
