The Quality Management Department exists to provide guidance to the Colorado Mental Health Hospital in Pueblo's efforts to meet or exceed customer expectations by maintaining excellence in patient care and to identify opportunities for improvement through analysis, interpretation, and dissemination of data-based information that enables hospital leadership to utilize resources in an efficient manner and to operate effectively under legislative mandates and administrative policies.
About the Position:
The Occurrence Reporter will assist with the implementation of the CMHHIP Critical Incident Reporting Program as outlined in CRS 25-3-109. This position reviews and analyzes incident reports s, using the standards of practice set by the Colorado Department of Public Health and Environment (CDPHE), and provides follow up for Colorado Mental Health Institute at Pueblo (CMHHIP), Office of Behavioral Health (OBH), and Colorado Department of Human Services (CDHS). Investigates and determines which events constitute a critical incident/occurrence and determines which of those must be reported to Executive Management in Denver, CMHHIP leadership, including the hospital CEO and the Deputy Director of Clinical Services for OBH and Colorado Department of Public Health and Environment (CDPHE). Collaborates with Program/Unit Leadership, Department of Public Safety (DPS) and appropriate Department Heads to respond to and investigate serious allegations that may require immediate response of an appointing authority to determine appropriate action to keep patients safe at all times.
Job Duties:
- Decides which reported incidents require further investigation and goes to treatment units, medical records, and other departments to complete investigations.
- Determines in consultation with the Occurrence Team manager, which incidents are occurrences that meet reporting requirements to (CDPHE) based on their guidelines in the Health Department Occurrence Reporting Manual. Many occurrence reports are not clearly defined violations of standards
- Analyzes trends in occurrence data and submits reports to executive staff for focused quality improvement projects
- Assists with the Quality Assurance Immediate Action and Safety Plan which involves collaboration with the QSS Occurrence team to monitor follow-up, assist in investigation and collect data for occurrences involving actual or allegations of patient mistreatment, abuse, neglect or exploitation (MANE). Data is utilized in completion of root cause analyses and/or Sentinel Event reports when indicated and to drive hospital-wide Quality Improvement planning
- Completes audits under the direction and in collaboration with the QSS QAPI team
- Collects and summarizes findings by aggregating data per regulatory guidelines
- Reports findings to the Director of Quality Management, and QAPI Supervisor
- Assists with completing data input and collaborates on system maintenance for QSS spreadsheets and databases for audits, focused audits or focused audits requested by legitimate special interest entities affiliated with CMHHIP and TJC Tracers
- Assists in determining the best method of data presentation based on the intended audience. Uses an established database in which results of all audits are maintained and through which comprehensive reports (including graphs and charts) may be extracted for the Director of Quality Management, Executive Committee or Program and Department Heads
- Maintains a system for tracking chart delinquencies with a goal of no delinquencies older than 30 days hospital-wide
- Monitors delinquencies and works with clinical staff to correct them within established timelines. Coordinates resolution of delinquencies with QSS QAPI team
- Provides reports on delinquencies to the Director of Quality Management and Quality Management QAPI supervisor
- Navigation in the BEHR in order to locate required documentation is challenging. There are often various places where documentation can be located. The position must function as a medical record Subject Matter Expert assisting staff in documentation as needed
- Utilizes knowledge of regulatory standards to evaluate patient care areas to determine if something needs to be addressed immediately to maintain patient safety. Contacts appropriate supervisory staff in the area to discuss said finding(s) and assists in determining options to correct deficiencies. Determines if policy or procedures need to be updated
- Decision making are determined on a case-by-case basis, dependent upon the assigned duty
- Other duties as assigned
Minimum Qualifications:
Experience Only:
- Six (6) years of relevant experience in an occupation related to the work assigned to
this position
OR
Education and Experience:
- A combination of related education (bachelor's degree in a clinical field of Counseling, Psychology, Nursing or Social Work) and/or relevant experience in an occupation
related to the work assigned to this position equal to six (6) years
Preferred Qualifications
:
- Bachelor's degree or higher in the clinical field of Counseling, Psychology,
Nursing, or Social Work. - Five or more (5+) years of experience working in a clinical or hospital setting.
- Behavioral health experience
- Experience investigating occurrences
- Prior utilization management experience
- Experience in writing professional reports
- Prior experience interacting with regulatory entities
- Previous experience in quality improvement measures
- Experience with data collection and analysis
- May work weekends or alternate shifts/schedules
- CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults Mental Health Institutes, Regional Centers, Veterans Community Living Centers)
- Shall not initiate or engage in sexual, social, personal or financial relationships with patients either during or any time after hospitalization, even if the patient requests or initiates such a relationship. All relationships with current or former patients must be disclosed on your application;
- Successfully pass all modules of the CMHHIP New Employee Orientation core curriculum to include English reading comprehension, relevant competency assessments and written/practical exams;
- Must be able to see and hear in order to observe, address and respond to potential breaches in safety
- Work in situations involving assaultive behavior
- Ability to read and comprehend the English language; effectively and fluently communicates in English, both verbally and in writing
- 2-step TB (tuberculosis) screening is required of all new employees upon hire;
- COVID 19 and Annual influenza vaccination required

Sign-On Incentive:
Upon approval, a sign-on incentive based on position, ranging from $2,000 to $7,000 (prorated if a part-time position), will be given in three payments: within two pay cycles of when the new employee starts at CDHS, 180-day anniversary and 270-day anniversary with CDHS.
