Shift Notes: 8:00A - 5:00P Mon - Fri (8 Hour Days) (24 Hour Guarantee)

Weekend Requirements: For Call

On-Call Requirements: 1 - 2 Weekends/Mo

Traveler Policy: Candidates' Permanent Address (Facility will not consider local candidates - 50-mile radius restriction)

Provides quality patient care in the home setting utilizing the nursing process and accomplishes indirect patient/office-related work following the policies and procedures of the Home Health Department.

QUALIFICATIONS:

• Experience in Home Health Nursing. Knowledge of Home Health
• compliance. Knowledge of patient management and techniques for physical intervention.
• Satisfactorily completed the hospital orientation program, and met additional credentials for
• Special care unit as indicated in the skills checklist.

Submission Requirements

• At least 1 year of experience - Required
• Recent traveler experience - highly preferred
• Reliable transportation - Required
• Please note Mileage is not reimbursed

• Skills Checklist, current to 1 year, pertinent to the specialty
• 2 Supervisory Reference Forms, taken within the last two years
• Licensure & Certifications (OK State License or compact & BLS - AHA, NIH - Preferred)
