Job Title: Palliative Care Nurse Practitioner (NP)

Position Summary:
We are seeking a compassionate Palliative Care Nurse Practitioner to provide patient-centered care for individuals with serious, chronic, or life-limiting illnesses. The NP will manage symptoms, support complex decision-making, and coordinate care in collaboration with interdisciplinary teams to improve quality of life for patients and their families.

Key Responsibilities:

  • Conduct comprehensive assessments and manage patients with complex chronic or life-limiting illnesses.
  • Provide symptom management for pain, dyspnea, nausea, fatigue, anxiety, and other distressing symptoms.
  • Facilitate goals-of-care discussions and advance care planning, including end-of-life decisions.
  • Collaborate with physicians, nurses, social workers, and other team members to coordinate patient care.
  • Educate patients and families about disease processes, treatment options, and self-management strategies.
  • Document patient care accurately in accordance with regulatory and institutional standards.
  • Participate in quality improvement initiatives, program development, and continuing education.

Qualifications:

  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Current NP licensure in the state of practice.
  • Certification as a Nurse Practitioner (family, adult-gerontology, or relevant specialty).
  • Experience or training in palliative care or hospice preferred.
  • Strong clinical judgment, communication skills, and ability to manage complex patient care.

Work Environment:

  • Outpatient clinic, hospital, or home-based palliative care settings.
  • Collaborative, interdisciplinary team environment with opportunities for professional growth and continuing education.

Requirements

Must maintain state certification and/or licensure.
