Opportunity:

New York City Integrative Palliative Care at Home seeks to provide an extra layer of support in the care of seriously ill patients. We work alongside patient's current providers to ensure they are receiving the time and attention they need to feel their best and live as well as possible for as long as possible. We focus on optimizing quality of life by addressing symptoms and other complexities related to illness and its treatment.

Unique opportunity to work with a Community-Based Interdisciplinary Palliative Care Team (Physician, Nurse Practitioner, Acupuncturist, Spiritual Care Counselor) focused on an Integrative approach to care for patients with serious illness and/or who are nearing the end of their lives.

The Social Worker will have the opportunity to grow with the company. Initially, you will be a contracted employee, paid per visit with bonus incentives for caseload growth/maintenance. You will have the option for eventual transition to full or part-time employment with benefits depending on company growth.

-Small patient ratios which will allow you to spend ample time with your patients and provide quality care (no more than 3 visits per day with a full caseload of 25-50 patients)

-Choose your own hours (while considering patient care needs)

-Determine your caseload volume

-Uniform provided

-Option for Telehealth care delivery depending on patient preference

Duties:

  • Assessing patients emotional and mental health and developing treatment plans to meet them
  • Asking questions and listening to help patients explore their thoughts and feelings
  • Help families make healthcare choices that are best suited for the patient
  • Conduct psychosocial assessments to determine the best palliative care solutions for patients
  • Liaison with patients and other medical professionals
  • Document patient's wishes and needs accurately
  • Contact agencies and alternative programs and speak on behalf of patients and their families
  • Provide emotional support, guidance and help resolve psychological crises
  • Intervene and resolve conflicts between families and the care providers
  • Provide patients and their families with helpful resources
  • Assist patients with their medical insurance documentation
  • Assist families with planning for funerals
  • Educate families on palliative/hospice care processes and update them on loved ones' care and well-being
