We’re looking for a dedicated and dynamic Patient Care Manager – Regional Float to join our team in Pennsylvania. In this pivotal role, you’ll oversee clinical operations across various sites, ensuring patients receive the highest level of care while mentoring teams and driving operational excellence.

What You’ll Do

  • Lead with Purpose: Oversee and coordinate daily clinical operations to ensure patients receive exceptional, compassionate care.
  • Empower Your Team: Mentor, coach, and support healthcare professionals to foster a collaborative, growth-oriented environment.
  • Drive Quality: Develop, implement, and monitor patient care policies and procedures that reflect best practices and compliance standards.
  • Ensure Compliance: Maintain adherence to Medicare Conditions of Participation and all state, federal, and local regulatory requirements.
  • Collaborate for Impact: Partner with interdisciplinary teams to streamline workflows and enhance patient satisfaction.

Requirements

What We’re Looking For

  • Experience:
    • Previous experience in a home care setting required.
    • At least two years of management or supervisory experience strongly preferred.
  • Expertise:
    • Solid understanding of accepted professional standards, Medicare regulations, and state/federal compliance requirements.
  • Skills:
    • Exceptional leadership, communication, and organizational abilities.
    • Strong critical-thinking and problem-solving skills.
    • A passion for improving healthcare delivery and patient outcomes.
  • Flexibility:
    • Willingness to travel across the PA region (3 days in-office, 2 days remote).

Required Certifications & Licensure

  • Registered Nurse (RN) with an active, current state license to practice nursing in Pennsylvania (or compact state eligibility).
