Ensemble Health, a division of Corus Health, is searching for a Patient Engagement Representative to join our team. This program engages referred members to assist with care coordination and identifying resource needs. The program goal is to assist members with navigation of the healthcare system and prevent unnecessary readmissions to the hospital. The optimal candidate will be a self-starter, highly organized, and an excellent communicator.

Duties & Responsibilities:

  • In collaboration with the Care Services Coordinator and/or Transitional Care Coordinators, performs in professional manner to provide support for programs including but not limited to Transition of Care and other population health management programs.
  • Referral processing in multiple systems
  • Perform telephonic outreach calls to members and document outcomes in all appropriate platforms
  • Assist with incoming calls, respond to need and/or redirect call as appropriate
  • Member schedule oversight and updates
  • Tracking process and outcomes
  • Run daily reports
  • Timely and appropriate communication with team members, supervisor, and/or leadership team members
  • Organizes the flow of information and communicates to team members so that members can be clinically managed timely
  • Make recommendations for improvement in all related areas.
  • Participates in meetings, training, workshops, etc. for the purpose of conveying and/or gathering information required to perform job functions.
  • Assumes additional responsibilities and performs special projects as needed or directed.

Requirements

Qualifications & Requirements:

  • High School Diploma or equivalent
  • Customer service experience preferred
  • Excellent communication skills
  • Capable of effectively navigating multiple software systems
  • Excellent organizational skills
  • Self-motivated to complete tasks
  • Basic knowledge of some medical terminology preferred
  • Direct patient care experience in a hospital or multispecialty clinic setting preferred
  • Bilingual in English/Spanish a plus
