Daily Responsibilities for Pediatric Behavioral Health Case Manager - RN / LPC / LCSW:
- Managing a caseload of 75-85 members on the healthplan ages 3-21.
- Conduct regular outreach (default monthly) and perform assessments yearly and as needed to determine needs, care gaps, current services, satisfaction of services, and provide education to members and families of members on the healthplan.
- Coordinate any needed or existing care and monitor care for satisfaction and member for progress throughout the year.
- Shift: M-F 8-5, overtime sometimes required
- Field in Austin, TX
- Clear and active RN, LPC, or LCSW license
- 1-2 years of case management experience required
- 1-2 years of working with pediatric population in Behavioral health setting
- Masters in Social Work/ Counseling; Bachelors only if RN
- Microsoft Office experience (excel, word, PPT, outlook)
- Clear and active drivers license, reliable transportation and insurance
- Case mgmt. experience 3+ years; Healthcare experience
- Ability to build rapport with pediatric members and families
- Knowledge of and experience with local resources under and not under healthplan for members
- Immediate enrollment in Health Insurance
