If you are passionate about making a difference in the lives of individuals and families, join our mission-driven team and experience a career well lived.*5000 New Hire Bonus*Early Intervention Registered NurseResponsible for coordinating treatment and implementing services for assigned clients as a member of a multi-disciplinary Early Childhood Services team.Essential Functions:Provide outreach services to children and familiesConsult with team members in the development of individualized plans and multidisciplinary services for assigned clientsMaintain client records completely and accurately to include IFSP's with full assessment of client functioning and goals, documentation of client progress in the program, records of required contact with collaterals and family members, documentation of arena assessments and documentation of DPH forms and regulations regarding intake and discharge of clientsComplete billing documentation according to schedule established by the companyAdhere to professional standards and guidelines according to their respective disciplinesKeep supervisor informed of progress of all clientsMeet direct service requirements based on part time or full time status, as designatedAdhere to clinical recommendations from clinical administrationMaintain confidentiality in all clinical mattersRemain compliant with billing and documentation in accordance with direct service requirementConduct visits with clients and relevant collaterals in natural environments according to the needs of the child and familyDevelop group curricula for assigned groups as applicableParticipate in team meetings, supervision and in-service training as requiredParticipate in program development and team goalsObtain necessary certifications as required (i.e. DEEC, EI certifications, etc.) and maintain licensure as applicableMaintain contact with site and clients by checking voicemail and emails dailyOther duties as requiredQualifications:Salary range-$75,000-$80,000Graduate of an accredited RN programCurrent MA state RN LicenseCurrent driver's licenseCurrent CPR/First Aid certificationExcellent assessment skills with the ability to exercise critical thinking in the clinical environmentEffective communication skills ability to communicate with team members, medical professionals, those we serve, and their familiesA reliable, responsible attitude and a compassionate approachA commitment to quality in everything you doJoin Our TeamIf you are passionate about making a difference in the lives of individuals and families, we encourage you to join Mentor South Bay's mission of helping everyone live a full and happy life.
