Job Title: Permanent - Physical Therapist (Acute & Outpatient)
Location: Grand Junction, CO 81505
Salary Range: $87,000 $100,400 annually + Full Benefits
Shift: Full-Time | Weekend Rotation Required

Position Overview:

We are seeking a skilled and compassionate Physical Therapist to provide high-quality care across acute and outpatient settings. This role includes evaluating patients of all ages, developing and implementing personalized treatment plans, and modifying interventions to ensure optimal recovery outcomes. The Physical Therapist will collaborate with interdisciplinary teams to ensure comprehensive patient-centered care.

Key Responsibilities:
  • Evaluate and assess patients to determine functional status and needs

  • Develop and implement individualized treatment plans based on evidence-based practices, clinical judgment, and patient goals

  • Modify treatment approaches to ensure effectiveness and goal achievement

  • Educate patients and caregivers regarding care plans, exercises, and recovery expectations

  • Produce timely, accurate documentation in accordance with regulatory and professional standards

  • Collect and analyze outcome data to assess patient progress and optimize interventions

  • Understand and contribute to cost-effective care by considering budgeting, billing/coding, and resource planning

  • Maintain consistent communication with patients, caregivers, and healthcare team members

Qualifications:
  • Bachelor's Degree in Physical Therapy from an accredited institution (Doctorate preferred)

  • Active, unrestricted Physical Therapist license in the State of Colorado (required)

  • BLS Certification (required within 90 days of hire)

  • Strong clinical reasoning, communication, and documentation skills

  • Experience in both acute and outpatient settings is preferred
