_{We are seeking a licensed Physical Therapist to join our rehabilitation team . The Physical Therapist will be responsible for evaluating, diagnosing, and treating patients in both inpatient and outpatient settings in accordance with physician referrals and best practice standards. This role offers the opportunity to work collaboratively within a dynamic care team to help patients regain mobility, independence, and quality of life.}

• _{Perform comprehensive physical therapy evaluations and diagnostic testing.}

• _{Develop and implement individualized treatment plans for patients of all ages.}

• _{Provide evidence-based therapeutic interventions in accordance with physician referrals.}

• _{Monitor and document patient progress, modifying plans of care as needed.}

• _{Communicate effectively with patients, families, physicians, and interdisciplinary team members.}

• _{Educate patients and caregivers on home exercise programs and rehabilitation strategies.}

• _{Maintain accurate and timely documentation in compliance with regulatory requirements.}

• _{Previous experience as a Physical Therapist Preferred}
