Job Title: Permanent Physical Therapist Outpatient
Location: Bad Axe, MI
Pay Rate: $37.78 $47.74/hour + Full Benefits
Schedule: 9:00 AM 5:30 PM | 72 hours per bi-weekly pay period

Position Summary:

We are seeking a dedicated Physical Therapist (PT) to join our outpatient rehabilitation team. The ideal candidate will provide patient-centered care by assessing, diagnosing, and developing individualized treatment plans focused on reducing pain, restoring function, and promoting mobility. In this role, the PT will work across outpatient, acute care, and swing bed settings.

Key Responsibilities:

  • Evaluate patients to identify functional deficits and develop appropriate, goal-driven treatment plans

  • Deliver one-on-one, evidence-based physical therapy care to patients

  • Work collaboratively with an interdisciplinary team to ensure comprehensive patient care

  • Educate patients and caregivers on therapeutic exercises and injury prevention strategies

  • Document assessments, treatment plans, and progress in the medical record system in accordance with professional and regulatory standards

  • Adapt treatment plans based on patient response and progress

  • Support patients in restoring, maintaining, and promoting overall physical function and mobility

Required Qualifications:

  • Graduate of an accredited Physical Therapy program Required

  • Licensed (or eligible) to practice as a Physical Therapist in the state of Michigan Required

  • Current BLS certification Required

  • 1+ year of experience as a Physical Therapist Strongly Preferred

Certification Requirements:

  • BLS (American Heart Association) Required
