_{The Physical Therapy Assistant (PTA) Home Health is responsible for providing in-home therapy treatments under the supervision of a licensed Physical Therapist. The PTA will carry out prescribed therapeutic exercises and modalities in accordance with the patient's individualized care plan, ensuring high-quality care in the comfort of the patient's home. The role includes a variety of documentation and coordination responsibilities and serves patients across all age groups, including infants, pediatrics, adolescents, adults, and geriatrics.}

• _{Administer physical therapy interventions as directed by the Physical Therapist.}

• _{Provide treatment using appropriate modalities and therapeutic exercises.}

• _{Accurately complete daily visit notes and discharge summaries.}

• _{Maintain and manage therapy equipment and documentation in a home setting.}

• _{Promote patient safety and independence in the home environment.}

• _{Communicate effectively with patients, families, and the interdisciplinary care team.}

• _{Oklahoma State Licensure as a Physical Therapy Assistant Required
  (Or valid Oklahoma Temporary Permit under direct supervision pending licensure)}
