Schedule: Full-Time | Day Shift
The Specialty Care Coordinator supports the Specialty Care Team by coordinating safe, efficient, equitable, and patient-centered care. This role involves managing patient referrals, scheduling, communication, and logistical needs to ensure seamless access to specialty services. The ideal candidate is organized, proactive, and excels in a fast-paced, collaborative healthcare environment.
Key Responsibilities: Patient Coordination & Support-
Coordinate care across specialty services to ensure effective and safe patient outcomes.

Assess patients' and caregivers' self-management skills and support autonomy through education, training, and referral coordination.

Communicate regularly with patients and caregivers to provide updates and facilitate access to services.

Prioritize referrals and patient outreach based on clinical urgency, safety, and care quality.

Schedule appointments, respond to clinic calls, and communicate with patients via phone, email, and WellApp messaging.

Generate patient letters and complete necessary documentation and forms.

Submit and manage patient travel requests, including Letters of Medical Necessity, escorts, and lodging arrangements.

Verify provider IT access, travel, and housing needs.

Collaborate with the Specialty Care Team to develop care plans and optimize healthcare outcomes.

Support front desk operations and ensure clinic coverage for all service lines.

Monitor clinic schedules, manage cancellations, fill open appointment slots, and ensure accurate patient preregistration.

Coordinate educational in-services for clinic staff when needed.

Collect and input coordination data into designated databases and complete reporting as requested by leadership.

Deliver the highest level of customer service and professionalism to patients, families, and staff.

Advocate for patients at the service-delivery level and respond promptly to their needs.

Maintain a clean, safe, and well-supplied working environment.

Report any missing or defective equipment and potential safety hazards.

Perform additional duties as assigned in a respectful and responsible manner.
Education & Certification:

High school diploma or equivalent Required
-
Associate's degree or 2 years of college in social work, healthcare administration, or related field
OR 4 years of relevant professional experience may substitute for formal education -
Basic Life Support (BLS) certification Required
Experience:

Minimum of 2 years of experience in a healthcare setting Required

Experience in a primary care or specialty clinic Preferred
Knowledge of:
