_{We are seeking a dedicated and compassionate Outpatient Physical Therapist to join our rehabilitation team in Chico, CA. This full-time, permanent position focuses on providing individualized, evidence-based care to patients with a range of diagnoses in an outpatient setting. The Physical Therapist will be responsible for evaluating, planning, and implementing treatment programs to improve patients' mobility, function, and overall quality of life.}

• _{Perform comprehensive evaluations and develop individualized treatment plans based on physician referrals.}

• _{Address impairments related to self-care, functional mobility, cognition, upper extremity function, and perception.}

• _{Deliver evidence-based interventions to support recovery and promote independence.}

• _{Provide caregiver/patient education to ensure safe and effective continuation of care.}

• _{Maintain accurate documentation of patient progress and clinical outcomes.}

• _{Collaborate with physicians and the interdisciplinary care team to optimize treatment plans.}

_Required:

• _{Graduate of an accredited Physical Therapy program}

• _{Current CPR certification}

_Preferred:

• _{At least one year of experience in an acute care, outpatient, rehabilitation, extended care, or mental health setting}

• _{Current California Physical Therapy License, or}

• _{California Limited Permit with intent to pass the next available board exam (employment contingent on successful completion)}

• _{Strong clinical judgment and technical competence}
