City of Hope's growing national system includes its Los Angeles campus, a network of clinical care locations across Southern California, a new cancer center in Orange County, California, and cancer treatment centers and outpatient facilities in the Atlanta, Chicago and Phoenix areas.The successful candidate:Schedule: Rotating shift times between 5:30 a.m. and 9:00 a.m. (majority of shifts start at 8:00 a.m.), with rotating weekends every 8 weeks.Job responsibilities:Provides both distributive and clinical pharmacy services across the ambulatory infusion center and inpatient oncology units under the supervision of the Director/Manager of Pharmacy.Serves as an essential member of the City of Hope care model, working directly with multidisciplinary teams in Medical Oncology and Surgical Oncology.Delivers high-quality, evidence-based care and provides discharge counseling to support optimal patient outcomes.Practices in alignment with hospital policies, practice agreements, and clinical guidelines.Manages disease states and monitors chemotherapy toxicities to promote safe and effective treatment.Performs pharmacokinetic services and ensures safe, cost-effective, evidence-based medication use.Completes inpatient rounding responsibilities and provides clinical support during patient care activities.Staffs the ambulatory infusion center as part of routine responsibilities.Verifies chemotherapy orders, dispenses medications, and provides drug information support as needed.Collaborates closely with oncologists, advanced practice providers, and infusion nurses to support coordinated patient care.Position qualifications:Minimum education:Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).Required courses/training:Completion of a pharmacy residency program preferred, or equivalent hospital pharmacy experience.Required certification/licensure:Current Illinois State Registered Pharmacist license.Preferred experience:Previous experience in oncology pharmacy preferred.City of Hope is an equal opportunity employer.City of Hope employees pay is based on the following criteria: work experience, qualifications, and work location.
