Location: Lapeer County

Job Type: PRN, Part-time, or Full-time

We are seeking a highly motivated Physical Therapist for our home healthcare patients in the Lapeer and Sanilac County region. The successful candidate will be responsible for delivery of high-quality care to our patients.

Responsibilities:

- Deliver high-quality care to patients in accordance with best practices and clinical guidelines.

- Ensure continued compliance with all practice standards.

- Participate in professional development and adherence to best practices.

- Collaborate with other healthcare professionals to develop and implement comprehensive care plans for patients.

- Implement physical therapy services in a variety of settings and practices

- Monitor and evaluate patient outcomes to ensure effectiveness of treatment plans, and adjust as needed.

- Participate in quality assurance and performance improvement activities to ensure continuous improvement of services.

Requirements

- Master's degree or doctorate in Physical Therapy from an accredited institution.

- Current state licensure as a Physical Therapist.

- Minimum experience in physical therapy role.

- Good knowledge of home healthcare regulations and standards.

- Excellent interpersonal, communication, and leadership skills.

- Ability to work independently and as part of a team.
