Physical Therapist Full Time Apply

Responsibilities
- Conduct assessments and develop individualized treatment plans for patients based on their physical needs and abilities.
- Provide in-home physical therapy services to patients in accordance with their treatment plans.
- Continuously assess and adjust treatment plans as needed to ensure progress towards patient goals.
- Communicate regularly with patients, family members, and other healthcare professionals to coordinate care and ensure effective treatment.
- Document all patient interactions, assessments, and treatment plans according to AdvisaCare standards.
- Adhere to all state and federal laws and regulations related to physical therapy practice.
- Maintain accurate records and reports of patient progress and treatment outcomes.
Requirements
- Current licensure as a Physical Therapist in the state of Michigan.
- A minimum of one year of PT experience, ideally in a home health care setting, is preferred.
- Strong understanding of physical therapy best practices and treatment methodologies.
- Excellent communication and interpersonal skills, with the ability to connect with patients and their families.
- Ability to work independently and manage a caseload of patients effectively.
- Must have a valid driver's license and reliable transportation.
