Schedule:
Monday Friday | 8:00 AM 5:00 PM (8-hour day shifts)
Compensation:
- Hourly Rate: $24.50 $27.56 (based on experience)
- Sign-On Bonus: $6,000
- Relocation Bonus: $2,000 (must relocate specifically to Lawton, OK-not eligible for nearby areas like Elgin or Cache; proof of new Lawton address required)

Position Summary:
The Physical Therapist Assistant (PTA) in Home Health is responsible for providing therapy treatments under the supervision and guidance of a licensed Physical Therapist. The PTA will implement approved treatment plans through therapeutic modalities and exercise, ensuring care aligns with patient needs and goals.
This role requires strong documentation skills and a collaborative approach to patient care, including assisting in maintaining departmental cleanliness and organization. Care may be provided across all age groups including infants, children, adolescents, adults, and the elderly.
Key Responsibilities:
- Deliver physical therapy interventions as prescribed by the supervising Physical Therapist
- Monitor and document patient progress, daily treatment notes, and discharge summaries
- Ensure compliance with all safety, quality, and regulatory standards
- Maintain a clean and organized work environment
- Communicate effectively with patients, families, and care teams
Requirements:
- Licensure: Permanent Oklahoma PTA License or Oklahoma Temporary Permit (must work under direct supervision until permanent license is obtained) Required
- Certification: BLS (Basic Life Support) Required
- Experience: New graduates are welcome to apply Will train
